About the CSO Online Forms Facility

You will need Adobe Reader ( Version 10 or higher is recommended ) to complete the PDF survey forms online. This software can be downloaded free of charge from the Adobe website.

What is the CSO online Forms Facility?

The CSO Online Forms Facility enables survey respondents to complete CSO survey forms online in a secure environment. This facility provides a fast and easy alternative to postal questionnaires. By using the CSO Online Forms Facility, you can submit your survey return via the Internet, 24 hours a day.

Selected CSO survey forms are available for online submission at present. This facility will be extended to other survey forms on a phased basis.

Completing the form:

Please Note that you will not be able to save any data which you type into the form. You may print your completed form if you want a copy for your records.


  • Use the TAB key (or mouse pointer) to move to the next field on the form.
  • Use the SHIFT & TAB keys (or mouse pointer) to move to the previous field.

Correcting errors: The eForm has in-built error/validation checks which appear in pop-up dialog boxes. Errors must be corrected before the eForm can be submitted.

Submitting your form:

After completing your form you may wish to review your responses before submitting the return. The button at the end of the form can be used to print a copy for your records, before sending the data to the CSO.

The Reset button will clear all the entries and allows re-filling of the form.

When you are happy with your responses, select the button to submit your return. If there are any outstanding errors on your return, the system will display an error page. You will need to correct all errors before the return can be submitted.

You will receive an acknowledgement page once your data is successfully transmitted. You may print this page for your records.
